| Terms and condition of the application: |

Applications are open to individual and organizations. |
» |
Payment to be made upon submission of application form. |

One time free medical checkup at GMCHRC only |
» |
Free benefits are not exchangeable with cash and valid for a year period only. |
» |
The card is not a credit card and it is not transferable. |
» |
The card must be produced as and when you avail the service. |
» |
The card must be presented for all billing purpose to avail agreed discount. |
» |
The validity of the card is stated on the card itself. |
» |
The Cardholder can use his Card at any of the listed hospitals to obtain services. GMCHRC however shall not be responsible if the Card is not honored at any of the aforesaid hospital for any reason whatsoever nor shall it be responsible in any way for the Services are rendered by listed hospitals. The two parties concerned must resolve any complaints from the Cardholder against the hospital. |

The lost, misplace or damage of. The card is held responsible by the cardholder. A replacement card can be issued at a charge of Dhs.25 |

Use of the card signifies acceptance by the holder of the terms and conditions under which the card is used and subject to amendments from time to time by the management of GMCHRC. |

Card is Valid for one year only. |
